Emilio Brunetti is a butcher by trade who runs his own store in Little Italy, Emilio's Butcher Shop. However, the ageing Emilio often finds himself at the mercy of gangsters wishing to bring Emilio into their protection rackets. In the past, Emilio was happy to pay the Corleone Family a weekly take and respected the Godfather. However, more recently, Emilio hasn't given a damn about paying the Corleones respect. He's been giving his kick-backs to the Tattaglias. As such, Frankie The Flush, a cardshark dealer has set up shop above the butchers store, running a small casino and paying the Tattaglia Family for protection and patronage.
Feared Corleone enforcer Luca Brasi decided to remind Brunetti where his loyalties lie, and uses the oppertunity to test the extortion skills of his new protege, Aldo Trapani. Trapani informs Brunetti of Luca's demand but to no avail. Taking matters into his own hands, Trapani reminds him why he needs protection in the first place. Trapani proceeds to smash up Emilio's stock until the unlucky butcher gave in. Trapani then finds out why the Tattaglia's are so interest in this butcher shop. Emilio has been forced into allowing an illegal gambling racket to operate above the store.
Later into the game, the Tattaglias are once again trying to force old Emilio to pay them. They send a soldier, a wise-guy named Mikey Saleri to 'convince' him to pay up. Saleri threatens to kill Emilio. However, Trapani once again keeps Emilio in the Corleone empire by murdering Mikey Saleri during an extortion.
